Health Service Partnerships

A Seamless Extension of Your Clinical Team

Delivering continuity of care from hospital to home.

The transition between care settings is often the point of highest risk for patients. Pindan Health Solutions partners with hospitals, government agencies, and community organisations to close this gap. We provide the clinical care and specialised workforce required to deliver hospital-grade care in the community, ensuring your clients remain safe, supported, and independent long after they leave your facility.

The Pindan Advantage: Why Partner with Us?

We don’t just accept referrals; we integrate with your objectives and your discharge plans. Whether you are a Local Health District needing to reduce bed block or an NGO requiring high-acuity nursing support, Pindan acts as a reliable extension of your own service.

Case Study: The Integrated Ecosystem in Action

Problem: Fragmentation. Solution: Pindan.

The Scenario:

A 78-year-old patient, Robert, is ready for discharge following a hip replacement and complications with a chronic leg ulcer. He lives alone, has limited mobility, and his overgrown property has been flagged as a falls risk.

The "Old Way" (Fragmented Care): Our Space

The hospital Discharge Planner would need to refer Robert to a nursing agency for his wound (if the Community Health team was already at capacity, which they invariably are), a separate cleaning contractor for hygiene, and a lawn maintenance service to clear the access path. Robert would also struggle to attend follow-up specialist appointments due to transport issues.

The "Pindan Way" (One Call, Total Care):

Pindan accepts the single referral and activates a coordinated, multi-disciplinary response

The Pindan Way

When Robert arrives home, a Pindan assessor conducts a Dynamic Risk Assessment, identifying that the front path is mossy and obstructed by overgrown bushes (a critical fall risk).

Our maintenance team is deployed immediately to pressure clean the path and cut back the vegetation, ensuring safe egress for Robert and the ambulance crew if needed.

A senior Registered Nurse visit is scheduled to manage the complex leg ulcer and monitor Robert’s post-operative vitals, escalating any signs of infection directly to his GP.

Robert is anxious about missing his specialist review. Our TLC Coordinator visits to set up an iPad, teaching Robert how to use it so he can attend his Telehealth appointment from his recliner, saving him a painful trip to the clinic.

Robert stays safe, his wound heals, and he remains connected to his medical team digitally. The Discharge Planner made one phone call, and Pindan handled the rest.
